1. KINDERGARTEN
The child must be 5 years old for Kindergarten by August 31 of the current school year.
A PSA authenticated certificate of live birth, as a documentary basis must be presented upon application for enrolment.
2. GRADE 1
The child must be six (6) years old by August 31 of the current school year. A PSA authenticated certificate of live birth, as a documentary basis must be presented upon application for enrolment.
The child’s Kindergarten Original Report Card is a requirement for enrolment.
The child must be able to pass the qualifying exams given by the school.
3. NEW STUDENTS
a. Present the following requirements to the Guidance Office on or before the date of testing:
Photocopy of PSA authenticated Certificate of Live Birth
Report Card: Original/Photocopy of Form 138 with grades up to third quarter if the student will take the entrance examination before the end of school year or before the release of cards. However, Original copy of the report card must be presented and submitted to the Guidance Center upon enrolment.
Fully accomplished Recommendation Form from the Guidance Center
If ESC Recipient from previous school, LRN, ESC Student ID Number and ESC School ID Number should be stipulated in the Certification. (applicable only to Junior and Senior High School)

b. The applicant is evaluated based on the following criteria:
Report Card (30%)
Entrance Exam (40%)
Interview (15%)
Reading Test (15%)
and must reach an average of(75%) to be accepted for enrolment
If for some valid reasons the applicant fails to present completely the necessary documents for admission, the applicant may be temporarily enrolled until such time that the requirements are fully accomplished. Before the First Quarter examination, the names of students who lack the necessary documents for admission will be posted at the Registrar’s Office. The documents must be submitted three days before the First Quarter Examination. This is for strict compliance.
4. TRANSFEREES
SMASN accepts transferees from other schools provided that the students have no failing grades in any subject as well as in conduct and that all the above mentioned requirements for new students are complied with. Depending on the circumstances of transfer, students who have undergone summer classes for failed subjects / repeaters may be admitted but they will be put under Academic Probation for close monitoring by the parents and the school authorities.
Because the Grade 6 and Grade 10 levels are terminal years, submission of Form 137 is a requisite. In case the student will not be able to submit the form 137 upon enrolment, he will be considered temporarily enrolled. The document should be submitted to the Registrar’s Office a week before the first quarter examination. Otherwise the concerned student will not qualify for Completion/Graduation and will not be issued any Diploma, Report Card, Recommendation Form from the Guidance Center, Certification for Completion/Graduation, Form 137 and other pertinent records from St. Mary’s Academy of San Nicolas should they be able to finish and pass this grade level.
5. FOREIGN STUDENTS
Present the following requirements to the Guidance Center on or before the date of testing:
Original copy of the Report Card
Recommendation from the Principal/Guidance Counselor of the previous school
A photocopy of the PSA authenticated Certificate of Live Birth
Alien Certificate of Registration (ACR)
Certificate of Eligibility for Admission: From the Department of Education
Form 137 from previous school
*Requirements e. and f. are not necessary if both parents of the applicant are Filipino citizens or if one or both parents have dual citizenship.
REQUIREMENTS FOR RE-ADMISSION TO SMASN (For Interrupted Enrolment)
Students who dropped out during the school year for valid reasons may be re-admitted the following school year without taking the entrance examination.
Students who transferred to another school or dropped for some time and would like to come back may be re-admitted after complying with the norms for admission of new students.
CONDITIONS FOR NON-ADMISSION TO SMASN
The following are conditions for non-admission in SMASN:
Two-time suspension of at least 3 days
Commission of grave offenses
Failure to take summer classes for failed subjects without valid reason
Failure to comply with the requirements of the Probation Agreement - academic or disciplinary
Non-payment of school accounts without prior agreement with the Finance Officer / Directress
SMASN students who are retained in the same grade level (due to psychological implication)
